Physical forms will need to be turned in separately via fax, email or at the Business Office. As usual, physicals are valid for 2 years and are required for all athletes including Dance and Cheerleaders. A student is ineligible to participate in any sport or activity until ALL of the forms are completed and on file in the Business Office. Students must also purchase and ASB sticker and pay the transportation fee for each sport/activity they are participating in BEFORE they are eligible.
